Forms needed before season start
2008 Registration Form	Please fill out completely and hand it at registration
2008 Physical Form	This form must be signed by family doctor and handed in before equipment is handed out. No child will be allowed to practice if not on file with the league.
2008 Physical Fitness Forms	This form is to be turned into coach or advisor at the end of the first week of practice.
2008 Volunteer Application Form	The Volunteer Application Form is mandatory for any person who is interested in coaching or volunteering for the league.
BYFL Membership Application Form	If you are interested in becoming a BYFL member, please fill out this form and return to League Secretary
Equipment Care and Instructions Sheet	Please keep on hand for maintenance and care of uniforms and equipment.
