All BYFL volunteers must fill out a volunteer form, thereby submitting to a background check, conducted the the BYFL Board of Directors. Thisis for the protection of the youth involved in our program and is mandatory. All volunteers are required to fill one out each year.
All Players and Cheerleaders must have a 2010 registration form filled out and submitted with registration money paid, prior to participating in the 2010 season.
NO equipment will be handed out to any player or cheerleader without a 2010 physical form on record with the league secretary. A 2010 physical from another sport will be accepted.
This form is for the parents to inform their child's head coach/coaching staff of any medically necessary information the parent would like the staff to know about. Typiccally, this is for allergies, disabilities, etc. All information is to be kept in confidence.
The BYFL is only as good as it members. Therefore, the BYFL is constantly looking for members, whose responsiblities will include electing new Board of Director members, assisting the league in various function, and promoting the league as a community organization. $2.00 and some of your time can change a child's life.
2010 INJURY FORM
The BYFL provides all of its players and cheerleaders with a ZERO deductible insurance program, that provides reimbursement to any player or cheerleader that is injured during any BYFL practice or game. It is a 6 page document that must be filled out by a BYFL Executive Director (President, Vice President, Treasurer, or Secretary), the injured party (or Parent/Guardian), and coach or advisor. All injury documentation from the injured healthcare provider will be required, so please have copies available when submitting to the league's insurer for reimbursement.
