following steps to submit your claim.

Once you have downloaded the forms, please
see the instructions below:

Parents

Fill out page 3
(Proof of Loss)
o
Once the forms
have been completed, it is the parent’s responsibility to fax all forms
once received to:

Football
Injury
o
Coach must
fill out and complete page 4 (Football-Injury Report)
·
Cheerleading Injury
o
Advisor must
fill out and complete page 5 (Cheer-Injury Report)
·
League
Official will fill out page 2 (Policyholder’s Report)
All injuries must be reported to the head
coach or the head advisor immediately!!!!
